File a Claim

To qualify for a settlement payment, you must submit a completed and signed Claim Form by August 3, 2026. You can submit the claim form in two ways: 

Online: You may electronically submit your Claim Form using the link below. You will need the Claimant ID and Unique PIN found on your Postcard Notice. Claim Forms submitted electronically through the settlement website must be submitted by 11:59 p.m. Eastern Standard Time on August 3, 2026.  

Mail: You may choose to fill out your Postcard Notice completely, detach that portion, and mail it to the Claims Administrator at:

Claim Forms sent by U.S. Mail must be postmarked by August 3, 2026. Mailed Claim Forms that are not postmarked or are postmarked after that date will not be considered for payment. Please retain the remainder of the Postcard Notice for your records.
